US medical bill or denied claim? Upload the bill, the EOB and the denial letter. Get the errors found, the dollars at stake, and the appeal letter and call scripts to fight it.
Just a few details to get started
PDFs from your insurer's website read best. A scanned PDF can come through blank; a clear JPG photo of each page works instead. You can also paste the text.
A claim denied for "medical necessity". A hospital bill that does not match what your insurer says you owe. An out-of-network bill for an emergency you had no say in. Errors and appealable denials are common, and most people pay because the paperwork is exhausting. Upload what you have: the itemised bill, the Explanation of Benefits (EOB) from your insurer, and the denial letter. Say what happened and, if you know it, what kind of insurance you have. The app reads the documents line by line - dates, CPT and HCPCS codes, billed and allowed amounts, what the plan paid, the denial reason codes - and checks them. It looks for duplicate charges, lines billed above what your EOB says you owe, adjustments the provider is supposed to write off, balance billing that may fall under the No Surprises Act, prior authorisation that was the in-network provider's job, and coding that looks inconsistent. **Only what the documents support is flagged**, and every dollar figure is worked out in code from the numbers on your paperwork, not estimated. The first read shows the issues found and the potential dollars at stake. The full pack adds a formal internal-appeal letter, a letter to the billing office (disputing the errors, asking for the itemised bill if you do not have one, and asking them to hold the account), call scripts for the insurer and the billing office, a deadline tracker with the typical appeal windows for your plan type, the external-review step, and what each code on your paperwork means. **A note on PDFs.** A PDF EOB downloaded from your insurer's website usually reads well. A scanned or photographed PDF often has no text in it and comes through blank; if that happens you are told which file, and a clear JPG photo of each page works instead. For US health insurance only. **This is not medical or legal advice**, and it cannot promise your bill or a denial will change. Deadlines are shown as typical windows; the one printed on your own notice is the one that counts.
